If you are planning to apply by mail, you may download a PDF of the application here.

A $35 (US dollars) processing fee is required. This fee for mailed-in applications is payable by check, money order, or the PayPal link, below. International applications can only pay via PayPal, checks or money orders will NOT be accepted. Those paying via PayPal must include a copy of the receipt with the application.
